How do I report an injury on the job?
It is important to notify your employer immediately after you suffer an injury. Be sure you fill out all of the necessary paperwork at that time as well. A Workers' Claim for Compensation must also be filled out and sent to the Division of Workers' Compensation at 633 17th St., Suite 400, Denver, CO 80202.
Am I entitled to collect lost wages?
If you have missed more than three consecutive days from your regular work schedule as a result of your injuries and you have a written note from a doctor stating that you are unable to work, you are entitled to compensation. Your compensation will be equal to two thirds of your average weekly pay prior to the accident.
What if I am released back to work on modified duty?
If you are released for modified duty at your job, you are entitled to receive Temporary Partial Disability Benefits to make up for missed or lowered wages during your recovery.
What if I am released back to my job too soon?
If you have been released from medical care, you are legally entitled to a second opinion. A Level II accredited physician will examine you and your rate of impairment will be rated. Then a date of Maximum Medical Improvement will be determined.
